Psilocybin services in Oregon are showing promising results outside the controlled setting of a clinical trial. A new study led by Oregon Health & Science University (OHSU) found that people who used state-regulated psilocybin services reported improvements in anxiety, depression, PTSD symptoms, mental well-being and life satisfaction.

Many participants also described the experience as highly meaningful, while serious physical health problems were not reported.

Published in JAMA Network Open, the research is the first multisite study to examine Oregon’s legalized psilocybin service system in real-world conditions. The findings offer a closer look at how people respond to psilocybin when they receive services through licensed providers rather than research trials.

Study Tracked Real-World Use

The research included 346 adults who took part in a single psilocybin session with licensed facilitators between November 2024 and June 2026. The sessions took place at 24 licensed service centers across Oregon.

Researchers collected information before the session and again one week, one month and three months later. Participants used a web-based reporting system to describe changes in their mental health and overall experience. About 90% continued reporting their outcomes through the three-month mark.

Lead author Todd Korthuis, M.D., M.P.H., a professor of medicine in general internal medicine and geriatrics at the OHSU School of Medicine, said the safety results were similar to those seen in controlled psychedelic research.

“Safety was very similar to what we’ve seen with psychedelics in clinical settings,” Korthuis said. He added that the findings matter because researchers have questioned whether safety seen in tightly controlled trials would also apply to real-world psilocybin use.

Oregon voters approved the state’s nonmedical wellness model in 2020. Licensed psilocybin services became available to adults age 21 and older in 2023.

Participants Reported Improvements

The results showed that many participants continued to report positive outcomes after their single session. At one month, 91.5% said they had benefited from the experience. Another notable finding was the personal importance participants placed on the session: 64.9% ranked it among the 10 most meaningful experiences of their lives.

At three months, participants reported lower levels of moderate-to-severe symptoms linked to depression, anxiety and PTSD. They also reported better mental well-being and greater satisfaction with life.

These results are notable because the Oregon program is not designed in the same way as a medical clinical trial. Only about half of the study participants had moderate-to-severe depression symptoms before receiving psilocybin. Still, many reported mental health and wellness improvements afterward.

Korthuis said the distinction between a medical model and a wellness model does not necessarily mean people cannot experience mental health benefits. He also stressed that people with serious mental or physical health conditions should consult health care providers before seeking psilocybin services.

Few Harmful Experiences Reported

The study found a relatively low number of reported adverse experiences. Only seven participants described the experience as harmful three months after taking psilocybin.

Four first-time users experienced behavioral reactions serious enough to require medical attention. However, no participant reported a serious physical health event connected to the session.

Korthuis noted that psilocybin is not considered appropriate for people with psychosis or bipolar disorder or for those who are pregnant. The study population may also have been healthier overall than some groups enrolled in clinical research, which could partly explain the low rate of adverse events.

Senior co-author Adrianne R. Wilson-Poe, Ph.D., a scientist at the Legacy Research Institute and co-director of the Oregon Psychedelic Evaluation Nexis (OPEN) with Korthuis, said the research provides an important look at the safety and mental health outcomes of people using these services in everyday settings.

“This paper is going to be cited for decades,” Wilson-Poe said. She described it as the first large-scale effort to monitor safety and mental health symptoms among people using psilocybin services in the real world.

OHSU Plans More Research

The study is part of a larger research effort examining Oregon’s regulated psilocybin system. OHSU received a $3.3 million, five-year federal grant earlier this year to study whether psilocybin could affect substance use among people seeking state-regulated services.

The project received approval from OHSU’s Institutional Review Board for its research platform. Wilson-Poe said researchers can also adapt the system to study other psychedelic substances and programs in states beyond Oregon.

OPEN has already expanded into Colorado, which became the second state to approve regulated psilocybin services. That expansion could allow researchers to compare outcomes between states using similar methods.

Wilson-Poe described the approach as providing an “apples-to-apples comparison,” giving researchers a clearer way to study how different regulated programs perform.

Cost Remains a Barrier

Access to Oregon’s psilocybin services is not equally available to everyone. A major issue is cost, with sessions involving state-licensed facilitators ranging from hundreds to thousands of dollars.

Location can also affect access. Several Oregon counties have chosen not to allow psilocybin service centers. Some local officials have raised concerns about psychedelic safety, including questions surrounding both therapeutic and recreational use.

More than 20,000 people have accessed licensed psilocybin services in Oregon, according to data tracked by the Oregon Health Authority. People seek these services for general wellness, personal insight and potential changes in how they view their lives and experiences.

OHSU President Shereef Elnahal, M.D., M.B.A., said the early findings could help policymakers assess programs involving psychedelics and mental health. Elnahal previously served as undersecretary of health at the U.S. Department of Veterans Affairs.

The study will also be presented at a Psychedelic Innovation Summit in Portland co-hosted by OHSU and the Healing Advocacy Fund.

What the Findings Mean

The research does not establish psilocybin as a treatment for depression, anxiety or PTSD. Instead, it provides real-world data on how people respond to state-regulated psilocybin services outside traditional clinical trials.

Because participants were not randomly assigned to psilocybin or another treatment and reported their own experiences, the findings show an association between psilocybin use and reported improvements rather than proving that psilocybin caused those changes.

The study also offers a wider look at Oregon’s regulated program. It found relatively few serious adverse events during the study period. Still, several questions remain. These include long-term safety, cost, access and which people may benefit most.

Future research, including OHSU’s planned work on substance use and comparisons between Oregon and Colorado, may help clarify these issues. Together, the findings give researchers and policymakers additional evidence as regulated psilocybin programs continue to develop.
